Consent Agreement for Holistic Animal Healing Treatment

 

I, the undersigned, acknowledge the following:

1. I understand that all medical diagnoses and treatments for my pet should be performed and recommended by a licensed veterinarian.
2. I recognize that holistic animal healing and Reiki are intended to reduce stress, promote relaxation, and restore balance to my pet’s energy system. They are not substitutes for any prescribed veterinary treatment.
3. I am aware that the benefits of each animal healing session may vary and may not always be immediately noticeable in my pet.
4. I hereby give my Holistic Animal Healer/Reiki Practitioner permission to conduct sessions on my pet with the goal of helping my pet relax and achieve balance.
